Post Permanency Services

If you have finalized an adoption or PLC in the past, and are feeling overwhelmed, frustrated, or lacking resources, you are not alone! Children's Choice, Inc. offers Post-Permanency Services to Pennsylvania families in order to ensure that they are provided the support needed following the finalization of an adoption or PLC. These services are made available by the Pennsylvania Statewide Adoption and Permanency Network (SWAN), in order to encourage positive lifelong experiences for families. The following three categories of Post-Permanency Services are available:

Case Advocacy:

This service is built to ensure that families receive access to the services designed to meet their needs following their permanent commitment to a child/children. During this process, a Children's Choice permanency worker will work closely with your family to identify each individual's strengths and needs, establish a family support plan, coordinate services to meet the family's needs, and facilitate collaboration between various service programs. Your permanency worker can advocate for you in many different ways, including collaborating with individual/family therapists, attending IEP meetings, and encouraging family interaction through home visits and family activities. The main goal of Case Advocacy is to keep your family intact, prevent disruption, and increase family cohesiveness.

Support Groups:

Do you ever feel like you are in need of interaction with other parents who have gone through the PLC/Adoption process? Attending a support group can help build relationships, form a community of mentoring, provide education, and offer socialization and recreational activities for your family. This support can come from trained professionals, or other parents who have gone through the permanency process, and understand what you are going through. Your Children's Choice permanency worker will discuss your support group options, and recommend an in-house support group, or one with a specialized topic to better meet your family's needs.

Children's Choice, Inc. understands the importance of building relationships with other families, along with forming a community of support and mentoring. A monthly support group is offered to our Post-Permanency families, in order to provide a safe space for families to share and grow. The support group is held on the third Monday of each month, and takes place from 6:00pm-7:00pm, at 3000 Market Street, Suite 200, in Philadelphia, PA. Respite:

This unit of service is designed to ensure that your family feels supported and empowered. Your Children's Choice, Inc. permanency worker will work alongside your family to focus on alleviating the pressures of parenting children with special needs. Through the respite process, families utilize planned "breaks" to be able to support a consistent level of parenting. The respite service is designed to help families maintain their cohesiveness and strong family bonds.
